Note: If you choose a monthly payment option, you will be charged every 30 days as long as your membership is active. If you choose an annual payment option, you will be charged again in 365 days, but you will first be notified by e-mail 30 days prior to renewal so you can let us know to not renew your annual membership if you choose to not continue.

Upon receipt of the application form you will receive a call from one of our licensed healthcare consultants confirming your purchase. A receipt will also be sent to you by e-mail. We will also send you a temporary Certus Health Membership Card, a temporary Wal-Mart Pharmacy Savings Card, and a membership guide. These will be sent to you by e-mail. The temporary cards will be good for 60 days, at which time permanent cards will be sent to you.
Please remember that you have to present your Certus Health Wholesale Card in order to receive the advertised prices from the participating providers. And when visiting any Wal-Mart pharmacy or Wal-Mart vision center, please remember to take your Wal-Mart Pharmacy Savings Card. Please bring your driver’s license or another valid proof of identification - the providers will check it with your membership card(s). Finally, when making an appointment, please inform the person taking your appointment that you are a Certus Health member.
You have 30 (THIRTY) days to try the Certus Health service 100% RISK-FREE. If you’re not satisfied during those first 30 days for ANY reason, just cancel the membership and you will be reimbursed in full.
